Refactor indexing

These changes have primarily been made with an eye on upcoming requirements
from the nhm's data-importer2 code where relationships will need to be built
and data augmented before indexing. To allow this to happen in an efficient
manner the chunks of mongo documents that require indexing need to be passed
around rather than the individual documents, allowing bulk extraction of data
from mongo rather than single documents.

The code is also now a little tidier generally though it still has some oddities
and decisions to be made.
