Update to Babel 7 beta

- Switch to scoped @babel/* packages
- Preset modules must now export a function
- @babel/preset-react now has a development option for jsx-self/jsx-source
- @babel/preset-stage-2 now requires a decoratorsLegacy option
- @babel/preset-stage-1 now requires a pipelineProposal option
- Use new useESModules option for @babel/plugin-transform-runtime for smaller webpack builds
- Use new useBuiltIns config for @babel/preset-stage-X
- Added a **/ prefix to co-located testignore patterns for module builds

- Switch from babel-plugin-react-transform to react-hot-loader for React HMR

Internal Babel-related stuff

- Drop constructor hack for UserError, as Babel 7's wrapper should handle instanceof
- Drop loose mode, as it currently breaks when extending builtins
- Use double quotes in test assertions against generated code
- Use Array.from() instead of spreading a Set to convert it to an Array
