Control update (#59)

* control_rate_comp -> control_interp_comp, component will be responsible for interpolating control rates AND values.  working with gauss-lobatto, radau needs work still.

* control_interp_comp working, all tests pass.
removed previuosly-deprecated phase.set_objective, all users should use phase.add_objective.

* removed control_input_comp and associated tests.

* refactored continuity_comp.  now use separate subclasses for GL and Radau, to avoid testing transcription in a top-level class.  continuity defects are now always computed (as long as num_seg > 1), but they are only enforced if requested by the user and not implicitly enforced via compressed transcription.  radau phases now use the same (order-1) for controls as GaussLobatto phases, for consistency.

* controls are now specified at subset control_input, even if externally provided.

* started work on a references section for documentation.  fixed double integrator example. added test of aircraft standalone example for coverage.

* setup.py typo

* coverage increase

* added release notes for version 0.9.0

* bumped version to 0.9.0
