AEIM-1315 - Refactor attribute extraction further

This is a pretty significant design change with a top-level theme of
eliminating the request wrappers and recasting them as attribute
extractors following a common interface. The key elements are:

- Define the persistent and enterprise ID concepts and attribute names
- Add a base class for attributes regardless of how they are extracted
- Set the expectation that all extractors will expose user_eid,
  user_pid, and client_ip
- Support finders for additional attributes in the base #all
  implementation
- Add a factory that can be configured as a service for an app, so
  there is an easy way to create the right attribute class consistently
