68 http headers setting (#94)

headers reworked. So access is sinatra like. User can now use headersPut: aDictionary to add multiple assosiactions asDictionary or single assosiations with headersAt: aKey Put: anAssosiation.
headers: methode removed since the complete disctionary can not be replaced htpResponse does not allow access. Also process speacial headers like conentType status contentLenght and cookies into their corresponding instanceVaraibles.
Http fieldnames are case insensitive as specified in RFC.

Added convinience func. to edit headers SinatraLike
4 header members (status, contentType, contentLength, cookies) have to be handled seperatly, because of squeak.

Tests:
Added a Middleware for globalHeaders. -> Therefore reworked middleware a bit.
Middleware now always have a Application settings as instance variable, so this might be used as a parameter.
