Improve robustness of recent constraint update migration

Some older CATMAID instances with databases that were created before
March 12 2013 (when the initial South migration was added) don't contain
some of the constraints referenced in migration 0039 or have them
available under a different name, because they only 'faked' the initial
migration. To make it possible to migrate those databases, migration
0039 has been more robust. Migration 0039 will now check for expected
constraints and rename them if required to the name used in the initial
migration. It will also create missing constraints.

Databases that already have all expected constraints with the correct
name will not be affected by this change. They were able to run
migration 0039 already and don't need any additional update.

A future migration should go through all constraints and make sure all
constraints are named like we expect.
