Convert PhpdocTrimAfterDescriptionFixer into PhpdocTrimConsecutiveBlankLineSeparationFixer

Closes #1686

Notes:
- there are situations when extra consecutive blank lines in PHPDoc are allowed, eg in Description - we respect that and not truncate them
- we don't truncate trailing empty line at end of docblock, as it's responsibility of `phpdoc_trim`
- we can change spec of rule, as it's not yet released
