Updates for OpenMDAO changes, normalizations (#42)

* started conversion of aircraft differential inclusion model to dymos.  added CRM aerodynamics interpolation.

* added more aero and propulsion components for the aircraft example.

* finished propulsion group with tests

* added flight dynamics group with test

* more work on aircraft example.  ode passes run_model in steady level flight.  need better logic in assert_check_partials, ignore rel error when nominal value is zero.

* more work on a standalone aircraft example problem

* aircraft example solves a simple IVP with a follow-on simulation

* more work on aircraft example.  Changed all references to metadata to options to comply with update to OpenMDAO. Removed vectorized components,
they now are part of OpenMDAO.

* ScipyODEIntegrator now uses the phase state_options, NOT the ODE state options.  This means that simulate() will use the units with which states and controls are declared.

* issue a warning instead of an error when providing options like lower and upper to nonoptimal controls

* aircraft example now picks altitude and airspeed to maximize range with 20000 kg of fuelin 2 hours of flight

* added aircraft_steady_flight example for testing.  defects are now computed wrt segment normalized time to help with scaling.

* more work on getting the aircraft example to converge to the correct solution

* adding a new continuity component that scales to phase tau space instead of time

* replaced old continuity component with new one.  Rate continuity is now assessed in phase tau space.

* removed all string format operations from compute and compute_partials methods in continuity comp

* removed dynamic_simul_deriv_repeats.  num_nodes -> vec_size for metamodelstructured.  component_metadata becomes component_options.

* skip MBI tests if unavailable

* more skips for MBI.  steady flight case is not working right still.
