fix #57 implement new MWSAdvanced()

- const MWS = require('mws-advanced');
  const mws = new MWS.MWSAdvanced({ ...credentials });
- done by doing some fairly creative re-binding, rather than re-writing
  large portions of lib and breaking the existing interface.
- tests indicate that this should continue working with the single-instance
  interface just as before, as well as allowing multiple interfaces to
  be created and used simultaneously.
