Move default plugins to constant, use in sphinx

nose2.plugins.DEFAULT_PLUGINS is a tuple of plugin names, so it's no
longer part of PluggableTestProgram. sphinxext checks membership in that
tuple before rendering the "enable plugin" docs.
Improvements to the docs on enabling a plugin.

Technically, sphinxext could have grabbed
PluggableTestProgram.defaultPlugins, but that's not as clean.
