SERENE-827: Provide streamed upsert of nodes into ElasticSearch (#57)

* Step 1: Refactor in-memory ingestion to be more stream-like.

* Step 2: Implement streamed EPG ingestion.

* Fixed integration test (ordering problem). Also started using mapping constants.

* Fixed integraton test failure by using a query returning fewer results than pagination limit.

* Do not bother to initially fill node cache.

* Turned off refreshing for bulk inserts and updates.

* Reduced ElasticSearch java memory to 1G.
