Enable code coverage support (#79)

* Enable gcov and lcov based code coverage support.

Support generation of gcov/lcov based code coverage results, e.g.:

  ./configure --enable-code-coverage
  make check-code-coverage

* Integrate support for the coveralls.io service.  The coveralls.io
  badge in the README.md documentation will now reflect code coverage
  results for master branch.
