Refactor directory structure and test runner

Rename `test/pakertaja.js` into just `test.js`, as the directory most
likely will never contain more than a single file, so there's no point
keeping a directory called `test` in the repository.

Update some of the dependencies to their latest versions and use `nyc`
to collect code coverage and produce coverage reports.
