add AR type/identifier info to more AR exceptions

- Added test for the HistoryConflictError and its properties.
- The CommandRevisionError now has the AR type prefix as well as
  the AR identifier and AR revision (like the HistoryConflictError).
- The HistoryEmptyError now has the AR type prefix as well as
  the AR identifier and AR revision (according to the command).
