Add locking to prevent various race conditions

We must be very careful when reading and updating persisted decrees such as
highest_proposed_decree, promised_decree,  and accepted_decree because these
functions may be called concurrently on different rounds of paxos. Otherwise it
is possible to accept conflicting values and end up with a corrupt ledger file.
