configure.ac: Added cc -Wextra Flag

Added cc -Wextra for extra compile-time source checking.

Corrected random_get_uint*() function comments uncovered by -Wextra checking.
That is, on error, these functions return 0 instead of random data.

Added *_STATIC_INIT macros to aid initializing structures flagged by -Wextra.
Added UNUSED_PARAM macro to remove unused parameter warnings.

Fixed or excluded all new warnings.

Fixes #406.
