Remove spinner and use angular-loading-bar (#451)

* Remove spinner and use angular-loading-bar.

Using a angular-loading-bar to handle display of progress when making htttp calls
this way there is no need to wrap the call in the spinner function.

Also changed the spinner image to use an svg so it looks better on hi-res screens.

* Unit test.

* Test resetProgramFields.
