Refactor linear trace operations; v0.7 deprecation; README

- Refactor linear operations on traces:
  - Add methods to +, -, * and / to act on combinations of SACtr
    objects and arrays/scalars.
  - Simplify add!, multiply! and divide!, and add subtract!.
  - Stop creating their copying counterparts automatically, as
    the method signatures are important.
  - Implement +, -, * and / for pairs of SACtr.
- Update setindex! so that setting :t will automatically update
  trace headers.  Note this does not work if the caller uses
  broadcast to do this.
- Fix another undef v0.7 deprecation warning.
- Update README.md to reflect the above and remove unnecessary
  advise on adding dependencies, as they are pulled in when
  doing a Pkg.clone.
