Test order resources (#7)

* created resorces for poc lab module

* updates and added status for the request and result orders

* more updates

* added testResult find by Patient

* more updates

* fix testRequest, some of the Test Concept was missing due non existence of reference for category concept

* package organization and updates

* once I have changed the attributes, had to change the query string

* created integration test for testRequestService

* added integrations tests for pocHeuristicService

* created unit test for service testOrderService

* fixed TestorderValidator, should retrieve only actived orders

* changed the way to retrieve the testConcept, now using a static mapping with de uuids of the test concepts and theirs conceptset

* added logic for patient provenance and code sequence in test order requesition and vizualized code sequence in test order result

* Removed patient provenance, since is no longer functional requirement

* one more fixe, should create only one ObsGroup for a set of exams belonging to the same category

* were comming only 50 TestRequest due limit size defined , so changed to AlreadyPaged results
