Add a new structured data type RowsOfFieldsWithMetadata.  (#66)

* Add a new structured data type RowsOfFieldsWithMetadata. Hide metadata in table-type formatters, but include it in raw formatters.

* Brief documentation.

* Adjust for AppVeyor tests.

* One more try at making Windows test comparisons work

* Allow field reordering et. al. to work with structured data with metadata.

* Add a NumericCellRenderer

* Add a HumanReadableFormat marker interface to formatters, and use that as an indicator in the NumericCellRenderer.

* Only run phpunit tests on Appveyor.

* Update changelog
