Simulation recording (#10)

* fixed a typo in the readme, and removed default values for partials in the brachistochrone ode

* simulate now records an sqlite database by default, using a system recording and the standard OpenMDAO recording format.

* one more go at getting radau to converge min time climb on travis

* fixed a bug involved with populating values of control rates when saving simulatoin results

* renamed f_rhs to ode.  when recording explicit simulation dymos now populates all outputs in the ode system in order to initialize any implicit variable outputs.

* optimizer_based_phase simulate now correctly assigns control variables

* simulate now properly initializes entire system when saving the results of an explicit simulation.  embed-code is having problems with redbaron, removed until addressed.  brachistochrone example updates.
