Avoid silently overwriting existing elements in a list. (#170)

* (M) ygen/*
   - Previously if RenameXXX was called with a newK that already
     existed in the list, then oldK was overwritten with newK silently.
     Check for the existence of newK and return an error if it already
     exists.
 * (M) exampleoc/oc.go
   - Regenerate code with updated Rename template to allow for schema
     tests to check the error is returned correctly.
