Set Keycard up like Checkpoint for database and Rails integration

- Add a DB module that sets up the connection/Sequel models (of which
  there are none yet in Keycard)
- Add Rake task for migration
- Add Railtie for autoinitialization in a Rails context
- Set up Railtie to integrate with db:migrate, db:setup, db:schema:dump,
  and db:schema:load
- Change InstitutionFinder to use a named parameter for the database
- Prune `test_database` helper method and do setup directly on
  Keycard::DB in sequel_helper
- Mark the InstitutionFinder tests as using the database, so they get
  a transaction around each test
- Correspondingly, move the test data into an before each, rather than
  the before all
- Move the migration out of spec and into db/
- Add some .gitignore entries for log, schema, and sqlite databases
