change ThrottledUserAgent to stop throttling after the first succesful request for a given object

we sometimes end up doing followup requests within a given ua.get() request, for instance to
follow html redirects.  turning off the throttling after the first succesful request allows those
followup requests to work as expected, at the cost of requiring the programmer to create a new
user agent object for each throttled session (but that is our common use pattern any way).
