AEIM-860 - Add Ettin and basic settings files

Loading the settings in application.rb seems to be a slightly better
approach than loading in an initializer for three reasons:

1.  The settings are loaded before the Rails environment files, so we
    can do Rails config adjustments based on our settings.
2.  The load order of initializers is based on filename; this is a more
    precise way of controlling the timing and making sure that the
    settings are available to all of our other components.
3.  We can place the config on the top-level module trivially, since
    this is where it is canonically defined. This also decouples the
    settings from the Rails configuration.
