Build proper supervision tree (#185)

* revise supervision tree

* increase timeout, change node startup debug message

* remove mochiglobal module

* make db connection module responsible for calling start_link/1

* replace poolboy calls for fmke_db_conn_manager, that uses round-robin policy over pools

* start app, cowboy, poolboy pools and conn manager

* add clauses for *_norm variants

* increase timetrap value

* update type specs, add basic unit tests

* make linter happy

* improve test function names

* add test groups for normalized versions, wait for propper shutdown at the end of each group
