Complete _start_index write/read for dicts

Dicts based on Namelist containing _start_index metadata are now handled
correctly.

Data is popped off of the dict rather than from args; this is again
possible since __setitem__ is no longer handling start_index.

Second, we explicitly dump all start index values into the dict of
todict().  I am honestly not sure why these were never written.  (I
think I incorrectly thought that a vector could have a start index of 1
even if it were not explicitly set, but I don't think this is true
anymore.)
