resolve #3, #4 - more generic at-rule support

make the "MEDIA" rule more generic and rename it "AT_RULE" to make it
capture and process more general @at-rules

this may break other things, but the tests all pass so :shrug: - you use
regexps to parse stuff like this, you get to keep all the pieces

update the 02-io.t test to use Test::File::Contents rather than doing
that by hand, update the Makefile.PL to reflect this dependency
