stub out ok and err types, add a few doc comments

I'm thinking that `Result`s should contain an `OptionT` internally because I
can't think of any other good way to deal with someone passing an `undefined`
value or `null` value into `Result.ok()` and `Result.err()`.  I haven't fully
thought out all of the implications of handling it this way, but it's the best
thing I can think of for right now.
