Namelist default_start_index active at write

The `default_start_index` property of Namelist no longer updates the
internal `start_index` array.  Rather, it applies the default start
index at write time.  This prevents a continuous update of start_index
every time default_start_index is changed.

There may be some recursive issues with derived type lists and things
like that.  Tests are still lacking.
