Use SearchCaseForm instead of BaseCaseSearchForm

SearchCaseForm derives from BaseCaseSearchForm and seems to work
just fine. I don't see the reason of having an if statement that
switches between the two classes when clearly we can use only
one class.

Also adds some cosmetic updates to the code!
