improve vCard based avatar functionality

* introduce and use utils.gather_reraise_multi, which waits for
  all tasks to finish and raises an exception collection individual
  exceptions that occured in the individual tasks

* handle MUC presence correctly (MUC presence may carry avatar
  information for a full JID instead of the bare JID, with translation
  of IQ requests across the MUC service)

* prevent spurious avatar change events, by comparing the new and old
  avatar descriptors.

* mitigate the race condition between notifies and get_avatar
