Add the CSV formatter to the combinator

Running:

    ruby combine.rb --format csv journals.csv articles.csv authors.json > full_articles.csv

will produce a full_articles.csv file with contents:

    DOI,Title,Author,Journal,ISSN
    10.1234/altmetric0,Small Wooden Chair,Amari Lubowitz,Shanahan, Green and Ziemann,1337-8688
    ...etc.

Running:

    ruby combine.rb --format json journals.csv articles.csv authors.json > full_articles.json

will produce a full_articles.json file with contents:

    --format
    json
    journals.csv
    articles.csv
    authors.json

Running with invalid inputs or file formats will raise the same
errors as before
