up test coverage, fix issues revealed by tests

there's a lot of mocking going on here, such is the nature of this
distribution, but it means the tests are truer unit tests and we
don't have to do end-to-end testing (010_basic.t) to still test the
modules

update MANIFEST and Makefile.PL with new tests and dependencies
