Added script to generate docs (#166)

* Added script to generate docs

This removes the `make docs` target and instead adds the script
generate_docs.sh to do a clean build, install locally and run doxygen,
and the generate_markdown_from_doxygen_xml script.

* travis: use new generate_docs.sh script

* Fix markdown generator script to avoid index and namespace xml

* Fix docs in headers

* generate_docs.sh: fix comments

* docs: fix doxygen warnings

This fixes the warning "End of list marker found without any preceding
list items".

* plugins: fix result_str comments

* generate_docs: added option `--skip-checks`
