Optimize squareform's square matrix construction

Avoid breaking chunks into smaller pieces than necessary by taking
advantage of symmetry. Namely only construct one of the triangles in the
square matrix. After constructing it use that triangle and its transpose
to reconstruct the rest of the result. Using a second pass, avoids
breaking the chunks more than necessary. So should make computations
with `squareform` faster.
