feat(appEnv): simplified and renamed appEnv API

BREAKING CHANGE: The old app API has been simplified and renamed to appEnv: app.config.* becomes appEnv.*

The new name better reflects the intent and avoids naming conflicts with the express app. To avoid confusion, the singleton instance accessor in cf was also renamed, from getAppEnv() to cfEnv().
