Update based on code review

- Removed "pub" connection in some of ITSubscriptionImplTest.java.
- Removed try/catch when just getting the message handler.
- Updated javadoc for set/shutdown of thread pool.
- Moved check for sub.closed in worker just after getting the sub.
  There is no need to cleanup the sub's pending counts when sub
  is closed.
- Fix pool.getSize() to return 0 if no worker has been created
  yet (was not needed outside of tests though).
- Also in tests, pool is shutdown/reset in size which could have
  caused issue with index if pool was reset to a smaller size.
- Added unit tests for worker and thread pool.
