Rewrite documentation for CLI and config modules

Rewrite the documentation for modules `pulp_smash.cli` and
`pulp_smash.config`:

* Be consistent about using terms like "host," "service" and
  "application."
* More thoroughly discuss how a Pulp application may be spread across
  multiple hosts.
* More thoroughly discuss the reason for having both a `ServiceManager`
  and `GlobalServiceManager`.
