Fold/Unfold blocks emitted by the front-end and processed by the back-end (#36)

* Output the type of the invalid root location as well.

* Emit Fold and Unfold blocks. Minor cleanup and refactoring.

* Extract one more helper function out of the vertex AST processing.

* Add FoldScopeLocation helper. Move base entities to separate file.

* Add a sanity check to make sure Fold blocks have unique locations.

* Update MatchQuery to be aware of folded scopes.

Tests pass again, though the output of FoldedOutputContextField does not yet inspect the folded scope data.

* Rename root_location -> base_location.
