Note GitHub contributor ToS; add PR template

Closes #13.

Per mbland/cla#2, I've opted to note the GitHub Terms of Service
regarding contribution licensing rather than using my own Contributor
Licensing Agreement for now.

Also touches up links in CONTRIBUTING.md and README.md.
