Refactor cmd/bytemark into four packages:

* testutil, which contains BaseTestSetup and BaseTestAuthSetup
* app, which contains BaseAppSetup, GlobalFlags, Context and the With
function
* app/with, which contains what used to be called Providers for With
* main, which still contains all the commands and the bare minimum
  of startup code
