fix(reader): fix dictionary record batch reader, synthesize dictionary index metadata

Dictionary record batches only contain one vector entry, so match encoding id against batch id. Due
to a bug in dictionary index metadata (ARROW-1363), ignore the index metadata and synthesize our own
buffer layouts.
