Clean up pipeline API wrt specifying span overlap

- Instead of returning islices from on_sequence, span overlap
  is now specified by overriding the define_input_overlap_nframe()
  method.
- The overlap is included in both ispans and ospans, but is excluded
  in header['gulp_nframe'] and in the number of committed frames.
- This makes FdmtBlock somewhat cleaner, and changes the behaviour so
  that downstream blocks will inherit gulp_nframe without the overlap.
- Also noted a couple of remaining TODOs in pipeline.py and ring2.py.
