Save unit-test JSON coverage report under .nyc_output/

this commit makes the default coverage output from unit tests
to be JSON instead of lcov and saves it under .nyc_output/.
end-to-end tests will store their coverage reports in this same
directory so we can run nyc report on everything and submit it to
Coveralls.io
