ciao-controller: manage CNCI via a cnci controller per tenant

Change how we manage CNCI lifecycle by creating a CNCI controller
per tenant. This controller is responsible for transitioning the
state of the CNCI and managing which operations are allowed on
the CNCI. Allow CNCIs to be removed if they are no longer needed.
