Fix shadowed variables

 * Rename the following two types to CamelCase, this is consistent
   with other existing types:

    - `getFromContext` to `GetFromContext`
    - `bindElement` to `BindElement`

 * Rename MySequence property `authenticate` to `authenticateRequest`,
   this avoids conflict with the imported `authenticate` decorator.

 * Fix the code in `UserRepository.find()` to not reuse `key` name

 * Rename `CustomerController` private property `repository` to
   `_repository` to avoid conflict with the imported decorator
   `repository`.
