Macro support removed from tokenizer

I don't see how macros would ever be applied to namelists (unless it was
some hefty user-defined thing) so they've been removed.

If, for some reason, this is ever needed, it can be checked and applied
outside of the tokenizer.

This seems to produce a minor speedup (1-2%).
