Refactor sign::Signer

We now have these traits:
- sign::SigningKey: a contain for a signing key.  You can get
  the key type, and have it choose a signature scheme from
  those offered.  That choice is expressed in the form of a:
- sign::Signer: can produce signatures with a fixed scheme.

This removes any possiblity of calling `sign()` with an
unsupported scheme.
