Generate codecov-badge if codecov appears in .travis.yml or appveyor.yml

Additionally:
- The appveyor badge has been fixed
- Additionally, all badge-partials now have whitespace-control characters
  such that missing badges do not generate empty lines, but still all
  badges are on their own line in the markdown file.
- Tests for a project that has all badges configured (in order to ensure
  the last point)
